SUMMARY:
Under the direction of a licensed nurse, the Rehab Aide-LNA delivers compassionate care while achieving positive clinical outcomes and resident/patient satisfaction. LNA functions within the scope of practice of his/her license and according to facility policies and procedures.

Experience Requirements Must possess as a minimum, a high school diploma or equivalent. Must be a Licensed Nursing Assistant and maintain an active license in the state of NH. Must have a minimum of one (1) year of experience working as an LNA

Qualifications

Must possess current LNA license in the State of NH.

Responsibilities

Assist therapists with patient treatments and evaluations. Assist with transferring rehab patients to and from treatment locations. Assist in developing and maintaining rehab standards. Assist in interpreting rehab policies and procedures to personnel, patients, family members, etc. as necessary. Performs administrative requirements, as directed, such as completing necessary forms, reports, filing/organizing, calling families, reviewing therapist hours for payroll, etc. Coordinate therapy programs and activities with other departments. Maintain resident treatment records, patient files and progress notes as required. Assure rehab area is maintained in a clean and safe manner for patient comfort, convenience, and per facility policies and procedures. Clean and maintain rehab rooms and equipment throughout the day, including all wheelchairs, walkers, ADL equipment, etc. within the facility. Keep a record of and report any broken or worn equipment needing replacement. Assist with tracking rehab equipment and ordering needed supplies with DOR Direction. Assist with restorative dining as needed.
