Under the direction of a licensed nurse, the Licensed Nursing Assistant delivers compassionate care while achieving positive clinical outcomes and resident/patient satisfaction. LNA functions within the scope of practice of his/her license and according to facility policies and procedures.

Must possess current LNA license in the State of NH.


Provides patient care in a manner conducive to safety and comfort. Assists with all aspects of patient Activities of Daily Living (ADL’s) to include bathing, grooming, dressing, transfers, and ambulation/range of motion. Implements care according to patient care plan, and notifies Nurse of any changes in patient condition. Documents in patient electronic medical record (EMR) through Caretracker. Serves meals and assists as needed. Monitor and record intake. Obtains vital signs and weights, as directed. Provide simple skin care treatments as permitted. Uses standard precautions when caring for all patients. Adheres to Contact and/or Droplet Precautions as directed. Make beds daily, and change linens per protocol. Participate in the education of Quality Measures and work daily to achieve desired results for each individual patient.
