Physical Therapist - Part-time

Experience Requirements Prior experience in a skilled nursing facility preferred. Must be a minimum of 23 years of age.

Qualifications

Must possess a valid and current Physical Therapist license in New Hampshire. Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle patients based on whatever maturity level, they are currently functioning. Must be in good general health and demonstrate emotional stability. Must be able to function independently, have flexibility, personal integrity, and the ability to work effectively with patients, personnel, and support agencies.

Responsibilities

Perform PT duties as assigned within scope of practice under a physician’s order. Review referrals and physician orders to develop appropriate plan of care and treatment. Interview patients/residents and evaluate type of therapy most desirable for individual patient. Involve the patient/family in planning objectives and goals. Coordinate therapy with other departments. Participate in discharge planning, development and implementation of patient care plans, assessments and other duties as needed. Document progress of patient/resident by completing daily flow sheets. Assist in transporting patient(s) as appropriate. Perform administrative requirements such as completing necessary forms, reports, etc. and submitting to the administrator as required.
