Social Worker
Hanover Hill seeks a Social Worker to assist in the planning, organizing, developing, of the Social Services Department, to assure that the medically related emotional and social needs of the residents are met and maintained on an individual basis in our upscale, 124-bed skilled nursing and long-term care facility in Manchester, NH.

Qualifications
Bachelor’s Degree from an approved school of Social Work or related field. Minimum one year of experience in a social service setting.
Responsibilities
Assist the Director of Social Services in providing the support and assistive services of the department for the residents residing in the facility. Interview residents, family members, as necessary, and in a private setting, to obtain social history. Involve the resident/family in planning social service programs, objectives, and goals for the resident when possible. Refer resident/families to appropriate social service agencies when the facility does not offer the services for needs of the resident. Provide information to resident/families as to Medicare/Medicaid and other financial assistance programs available to the resident. Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs. Participate in discharge planning, development, and implementation of social care plans and resident assessments. Maintain an excellent working relationship with the medical profession and other health-related facilities and organizations. Coordinate social service activities with other departments as necessary. Assure that all progress notes charted are informative and descriptive of the services provided and of the resident’s response to the service.
